Continuing Care at Home (CCaH) programs provide the support and resources that older adults need to age in their homes. If you currently operate a CCaH program or are considering starting one, join other CCaH programs and experts at this virtual conference specifically focused on CCaH issues. There will be opportunities to network with other CCaH programs and get ideas and answers from experts in the field.
Robyn Stone, co-director of the LeadingAge LTSS Center @UMass Boston, and Amanda Young, executive director at The Wesley at Home, will be on hand June 12 from 3:30 to 4:30 p.m., to present findings from the most recent benchmarking survey and from Amanda’s dissertation research on the wellness coordination model in CCaH programs. There will also be a discussion of the future of research for CCaH programs and important data for programs to collect. Session participants will understand the importance of data and research, learn what data to track, and explore opportunities for future research to build the credibility of the CCaH model.
